Clients (those attending one or more paid sessions)

Information collected is used as follows:
to provide you with the agreed upon therapy in order to help you reach your goals.
to gather feedback used improve the quality of policies and services for all clients.
to provide clients with signposting to any information or guidance deemed helpful.

Enquiries:

Those who have made contact and/or participated in an initial consultation but have not proceeded with paid sessions - information is used to reply to queries, answer enquiries and offer suggestions regarding treatment and appointment booking.

The lawful basis to collect, process and hold data is by consent of the individual data subject. By contacting Pathways Hypnotherapy and/or submitting your personal data, you agree and provide lawful consent to using your data in this way.

Disclosure of your information

I will not share any of your confidential, personal, identifiable information with any third parties unless instructed or permitted to do so by you or required to do so by law or in special cases, such as a physical threat to you or others. In the event that I am legally compelled to disclose such personally identifiable or personal health Information to a third party, I will try to notify you unless in doing so we would violate the law or court order.

Where and how is your personal data stored?

Any digitally processed personal identifiable data collected is stored under password protected encryption. All written personal data (signed consent forms) and other sensitive data are filed and locked away in a safe location.

Retention Period

Clients :

Those clients attending one or more paid sessions - all data may be stored indefinitely but will not be stored for a period of less than 5 years, for the following reasons:
To provide continuity of service to clients who return for future treatments
To fulfil legal obligations should case notes be required by a court of law

Enquiries:

Those who have contacted us and/or participated in an initial consultation but have not proceeded with paid sessions - all personal identifiable and contact data to be stored for 1 year, allowing enquiry sufficient time to make a decision without pressure. If after the initial 12 months the enquiry does not become a client then the data will be erased within the next 12 months.

Your rights - access to information

The Act gives you the right to access information held about you. Your right of access can be exercised in accordance with the Act. Any access request may be subject to a fee of £10 to meet the costs in providing you with details of the information held about you.
